Default E2 levels

Who out here is knowledgeable about estrogen levels and injectables? I am on day 8 of my cycle (day 6 of stimming), using follistim. Was at 75 IU's for 3 days, 100 IU's for 3 days, and now back down to 75 IU's tonight. My follies are around 7-9 mm right now. I am concerned though because my doctor called today and my estrogen is 1031!!! I think it was like 70 or something at my last appt or at my baseline. She backed down on the dose. Does this mean I will overstim? Can your estrogen level remain stable while your follies continue to grow if you decrease your dose? This is my first cycle of injectables so I am questioning everything and trying to learn as much as I can

Wow, that does seem high! I know mine got up to 700 something by the time I was ready to trigger. I know the level is supposed to climb slowly as you progress while your follies grow. I'm surprised that your follies haven't grown more if the level is that high. I'm not quite sure what to tell you. I would def see what your RE has to say about that level, and I would ask lots of questions!! I think no matter what after a cycle of injectables most people tend to get cysts. Even though I got BFP I still have a HUGE cyst on my left ovary. I really hope this works out for you Jilly and that your body is able to even itself out. keep me posted!!

Thanks baby I love that you come over here still to help out, I find your responses so valuable! Well actually she measured one follie twice and the first time it was 8.9mm and the second measurement was 9.9mm, but everything else was around 7-8 I think. I was just shocked at the estrogen level skyrocketing! I go back in in 2 days so I will question her about it. I am just worried I will overstim and have to cancel the cycle. I know this was a risk when I started the cycle, but was really hoping it wouldn't happen.

Jilly - Rule of thumb is that each mature follie represents 200-250 E2. (Or is it 150-200? I forget, so hopefully someone will correct me.) Do you know how many follies you have? I wonder if you have a bunch of smaller follies that are causing the higher E2.

I would usually trigger when my E2 was 700-1300 on Follistim. I typically had 2-4 mature follies with those E2 levels. I don't think I ever had to lower my dose (just sometimes had to increase it), so I don't know if E2 will decrease if you decrease your meds.

Thanks EJ. I can't remember how many follies I had, like maybe 3 on each side that were around 7-8? And then the one bigger one that was around 9 or 10mm. I am hoping the little ones are putting off the E2 and they don't all develop. Today I noticed I am quite bloated despite going down on the dose. Ugh, this is starting to not be so fun

Jilly - I really hope your body evens itself out especially with going back to the lower dose. Maybe that higher dose was just too much for your body?? So you are back in on Friday? I hope it goes well!!

Jilly - It's possible to have them level off... Each little follie will put off some estrogen, but that doesn't mean they are ready for anything. They need to get to be at least 17mm before a doctor will trigger (though ones as small at 14mm can have a viable egg). My cancelled cycle, my estradiol was over 5000. I had about 6 follies that were 14mm or bigger (I think... might have been 7)... and then I had like 12 or so that were 12mm... and something like 40+ that were less than 12mm. (My RE didn't measure anything smaller than 12mm.) So, as long as you only grow a couple of those follies to the larger sizes, things should balance out. If it gets too high, your doctor will cancel. And if it stays that high, it does increase your risk of OHSS...

I had my e2 go up to 24,000 at one point..needless to say that cycle was cancelled.

There should be no problem in lowering the dose. It should be ok for your doc to coast now. It does not mean overstimming will occur. As long as she backed down on the dose and coast from here it should be ok. Good luck.
